SMS Terms and Conditions

These SMS Terms and Conditions (“Terms”) govern the text messaging program operated by Archstone Oral and Facial Surgery, 6063 Mission Street, Daly City, CA 94014 (the “Practice”). By opting in to receive text messages from the Practice, you agree to these Terms.
1. Program Description
The Practice sends text messages relating to your care and your relationship with our office, which may include: appointment confirmations and reminders; scheduling requests and changes; pre-operative and post-operative instructions and follow-up; responses to your inquiries; billing and account notifications; and internal operational alerts to practice personnel.
2. Consent and Opt-In
You consent to receive text messages by providing your mobile phone number to the Practice and agreeing to receive texts — in person, by phone, in writing, or through our website forms. Consent to receive text messages is not a condition of receiving care, goods, or services from the Practice.
3. Message Frequency
Message frequency varies based on your appointments and interactions with our office.
4. Fees
Message and data rates may apply. Charges are billed by your mobile carrier under your mobile plan. Contact your carrier for details.
5. Opting Out
You may cancel the SMS service at any time by replying STOP to any message from us. After you send STOP, we will send you one final SMS to confirm that you have been unsubscribed, after which you will receive no further messages. To rejoin, provide your mobile number and consent to our office again.

7. Carrier Disclaimer
Mobile carriers are not liable for delayed or undelivered messages. Message delivery is subject to effective transmission by your carrier and is outside the Practice’s control.

Information collected in connection with this program is handled as described in our SMS Privacy Policy. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Protected health information is governed by our Notice of Privacy Practices.
9. Changes to These Terms
We may update these Terms from time to time. The current version will always be posted on this page with its effective date. Continued participation in the program after changes take effect constitutes acceptance of the revised Terms.
